What is Support Coordination?

NDIS Support Coordination is a service that supports NDIS participants to get the most out of their support plans. We give you choice and control over the disability supports and services you access. We work closely with you and your family or carers to find the most effective way to achieve the goals in your NDIS plan.

While our Coordination service is run by Hunter Disability Services, our Support Coordinators are committed to providing nonpartisan support to find the best outcome for every individual.

What does a Support Coordinator do?

Support Coordinators manage processes related to your NDIS plan and help you access the disability services you need. We are here to:
  • Research and assess disability service providers, helping you choose one that best suits your needs
  •  Negotiate with disability providers on their services and price
  •  Provide financial advice and liaise with your Plan Manager, if you have one, on your behalf
  •  Arrange additional assessments to meet funding requirements
  • Help to resolve any problems or issues related to the NDIS or NDIA
  • Support you to have your NDIS plan reviewed in a timely manner.
HDS have a team of Support Coordinators that have a diverse skillset, each with MANY years of experience and qualifications in all facets of the industry. When you choose HDS to provide your coordination, we will always meet with you first to ensure we match you with the Coordinator that best suits your needs.

What is a Psychosocial Recovery Coach?

You can be considered to have a psychosocial disability or mental health illness if your condition significantly impacts your day to day living and your ability to participate in community life.

The National Disability Insurance Scheme (NDIS) offers support for people living with a psychosocial disability to aid in your recovery. This funding enables you to select the supports you need to achieve your personal recovery goals and lead a positive life.

Recovery Coaches combine the principles of recovery-orientated practice with coaching principles with the aim of assisting you to take more control of your life and to better manage the complex challenges of day-to-day living. Recovery Coaches will work with you collaboratively to assist you to live a full and meaningful life, and to build capacity and resilience through strong and respectful relationships.

This mental health service is designed to be able to maintain engagement through periods of increased support needs due to the episodic nature of mental illness.

Our Recovery Coaches will:

  • work with you to understand your NDIS plan, and to develop, monitor and review recovery plan goals.
  • provide coaching to you and work collaboratively with your family, carers, providers and other supports to connect you to a range of services.
  • work with you to build strong, respectful relationships.
  • assist you to take more control of your life and to better manage the complex challenges of day-to-day living.

All of our HDS Recovery coaches have either a registration with the Australian Association of Social Workers, Post Grad Psychology, Tertiary Counselling qualification, Cert IV Mental Health and/or lived experience.

If you have (or are seeking) funding for Level 2 Support Coordination or Recovery Coaching, get in touch with our team and we can help (02) 40 440 540.
